Clinical Assessment of Pregnancy Outcome after Laparoscopic Management of Endometriomas Infertile Patients

Abstract
Objective: To assess pregnancy outcome after laparosocpic management of endometriomas. Methods: Retrospectively, we reviewed 32 patients who underwent laparoscopic management of
endometriomas in infertile patients from January 1995 to December 1999. In this paper we describe the pregnancy outcome after laparoscopic management and followed during subsequent pregnancies. The minimal follow-up period was 24 months.
Results:
A total of 32 infertile patients with severe endometriosis were treated by laparoscopic management. Among these patients, a total of 18 pregnancies occured in 17 patients who became following surgery. One patient conceived twice. All patients had
severe
endometriosis(¥², ¥³) according to rAFS classification. The pregnancy rate was 56.3%(11/20) and, of the secondary infertility group, 50%(6/12). A significantly increased pregnancy rate was found for the first year compared to the second(61.1% vs.
22.2%). The mean time to postoperative management conception is 11.67¡¾6.6 months(range 3-36). The average age of the 32 patients was 26.3¡¾2.23 years(range 22-30). The average operating time was 56.4¡¾16.5 minutes(range 20-120). The mean
estimated
postoperative blood loss was 1.20¡¾0.54 gm/§£(range 0.2-3.2). Conclusion: Our results suggest that laparoscopic management of severe endometriosis enhances fecundity in infertile women.
